Referral Process:
How Our Referral Process Works

The referring partner fills out a Referral form to initiate the referral process. The form can then be sent by email or through our website.

The referring partner provides information about the patient, including their name, contact information, and reason for referral.

The referring partner may need to provide additional information, such as the medical history, to verify proper scheduling and treatment at DOC+

Once verification is complete, DOC+ will contact the patient to schedule an appointment at a time that is convenient for them.

Throughout the referral process, DOC+ maintains open communication with the referring partner to ensure that they are aware of the patient's progress and any updates regarding their care.

After a thorough examination and diagnosis, DOC+ will develop a personalized treatment plan tailored to the patient's specific needs.The Treatment coordinator will discuss the treatment plan along with the financial obligations. The referring dentist will receive a report detailing the treatment plan and with continual updates throughout the course of treatment.

DOC+ provides comprehensive follow-up care to ensure that the patient is on track to achieving their treatment goals and that any issues or concerns are addressed in a timely manner.

DOC+ provides feedback to the referring partner on the patient's progress and treatment outcomes to ensure that the referring partner is kept informed and up-to-date.
